This application, with the required recommendations, must be submitted to the Southern Maine Women’s Golf Association Scholarship Fund at 128 Warren Avenue, Portland, Maine 04103 no later than July 15, 2010. The scholarship(s) to be awarded are to be used solely for the purpose of furthering the applicant’s education (i.e., tuition, books, related expenses). Scholarship awards are paid directly to the educational institution. If for any reason, a recipient of any scholarship should fail to matriculate, she shall be obligated to return all scholarship funds to the Southern Maine Women’s Golf Association Scholarship Fund. By signing this document you, the applicant, agree to be bound by the terms of this contract.
